Donation Request Form

Neighborhood Credit Union is dedicated to building relationships in your neighborhood. As part of this commitment, we want to support the organizations that enhance the lives of our neighbors. To request a cash or in-kind donation from Neighborhood Credit Union, please review the eligibility requirements and donation guidelines below. Eligibility Requirements

• Donation requests to benefit an individual's cause or a single family's cause are not eligible.
• Donations must go to benefit a nonprofit organization with tax exemption status under Internal Revenue Service code 501(c)(3). However, requests from schools (private or public) or school-affiliated organizations like a PTA will be considered.

Donation Guidelines

• Request amounts may not exceed $500. If request is granted, check will be made payable to organization, not an individual.
• Neighborhood Credit Union requires a minimum 45-day notice for donation requests.
• Requests must be submitted in writing, accompanied by a Neighborhood Credit Union Donation Request Form and faxed or mailed to Neighborhood Credit Union at:

• Neighborhood Credit Union can grant only one donation request per organization, per calendar year.
• Donations must go to benefit residents of the Dallas Metroplex, specifically areas our branches serve (Arlington, Dallas, Duncanville, Mesquite, Rockwall).
• The organization requesting the donation must employ ethical methods of publicity, promotion and solicitation of funds.

You will be notified of the decision by Neighborhood Credit Union within two weeks, either by phone or mail.
